Visit your unaccompanied housing office to submit your completed forms.

Packages MUST be STAPLED in the following order (no loose papers):

• Completed and Signed Application: Must be fully filled out, Signature & Date must be included at the time of referral
• Copy of Orders: Must show PRD more than 6+ months out, and Fleet TEMPS data sheet may accompany orders for undesignated Sailors
• LES (Current End-of-Month LES): Required unless forward deployed (LES still must be verified before referral to PPV in eMH) and LES must not show any dependants and no BAH (except the standard $8.10)
• Chit: Bottom lined by the Commanding Officer. Exception: Carrier Personnel may have chit bottom-lined by Department Master Chief or above
• Sex Offender Addendum: Must be completed
• Proof of Financial Counseling: Only the page signed by both the Service Member and Financial Counselor is required
